At Flexout Health we are passionate about helping seniors stay mobile, safe & happy wherever they call home. We have experienced practitioners who are able to conduct both in clinic or home visits in the Albury-Wodonga and North-East Victoria region.

We are highly accessible & we are able to tackle barriers head-on to improve independence and increase the confidence of our clients. We work in collaboration with clients’ general practitioners and their healthcare team to develop a treatment plan based on their current medical status. Our goal is to implement a safe and effective program to help motivate and support our clients to ensure they have the necessary skills to improve and manage their health conditions safely and independently. Although most interventions are exercise-based, our team may also provide health and physical activity education, advice, support, lifestyle modification, massage and prescribe a variety of aids, depending on the participants needs.

Established by the Federal Government, My Aged Care is a service designed to make it easier for Australian seniors and their families to find information on government-funded aged care services in Australia.
You can contact My Aged Care directly by calling 1800 200 422 or through the website (www.myagedcare.gov.au) to assess your eligibility for government assistance. You will be asked a series of questions to determine your eligibility for aged care services and to gather some basic information. This will take around 10 minutes or more to complete. Please note, you will need your Medicare card handy as you go through this part.
To be eligible you must be aged 65 years or older (or 50 years for Aboriginal or Torres Strait Islander people) to be eligible for an assessment. You may also be eligible if you are:
- On a low income, homeless or at risk of being homeless, and aged 50 years or older (45 years or older for Aboriginal or Torres Strait Islander people).
- Finding that you are struggling to remember or perform daily tasks.
- Been diagnosed with a medical condition or you have reduced mobility.
- Experienced a change in family care arrangements or if you have suffered a recent fall.
- Have recently returned from a hospital admission.
Services provided by Flexout Health can include both in person (clinic or home visit) and non face-to-face services. Time spent by our practitioners including travel is billed as per the relevant fee schedule. For a copy of the fee schedule, please contact healthyageing@flexout.health.
The first step is to click the link below to complete the My Aged Care Intake Form. This can be completed by the My Aged Care client themselves, their guardian / power of attorney, or My Aged Care Case Manager.
